In an effort to streamline our process, we have introduced a new format:

If you are a TEACHER you are in the right place. Please fill out the Evaluation form below after your Artist visit. As soon as you fill out the form, and it is received at TCAA, we will pay the Artist. The alternative is a downloadable form that your artist/performer will give to you to fill out. Sign and hand back to the artist/performer right after their visit has ended. They will take care of delivering the evaluation to TCAA.

Note: If you do not know the email, address or phone number of the visiting artist, please refer to your registration/confirmation email that you received.

FOR ARTISTS ONLY:

As you know, we must receive an evaluation from the teacher before you can be paid. We prefer that the teacher fill this out online, but as we know, teachers are not always near a computer immediately after the class. To help facilitate payment in a timely manner, we are providing the online form as a printable document that you can bring with you to your session.

DOWNLOADABLE PDF OF EVALUATION—DOWNLOAD AND PRINT THIS DOCUMENT TO HAND TO TEACHER AT END OF YOUR CLASS. THEY MUST FILL IT OUT, AND SIGN. YOU MUST THEN DIGITIZE THE DOCUMENT AND SEND TO TCAA. (dona@tuolumnecountyarts.org)
