In addition, ARTIST and TEACHER have verified that there are sufficient funds to cover the cost of the proposed class, or classes in their school’s ARTS account. CHECK HERE

Copies of this form will be sent to you, the teacher, and ARTS staff. Please keep a copy of your email with this information, and please let the teacher know that they will need to refer to this email to fill out the evaluation.

As you know, we must receive an evaluation from the teacher before you can be paid. We prefer that the teacher fill this out online, but as we know, teachers are not always near a computer immediately after the class. To help facilitate payment in a timely manner, we are providing the online form as a printable document that you can bring with you to your session.
